Dr. Raymond Fulp, DOOrthopedic Spine and Extremities
Dr. Ray “Trey” Fulp is an Orthopedic Surgeon born and raised in Oklahoma. After graduating High School, he joined the United States Marine Corps. Upon his discharge, he attended Oklahoma Central State University for his undergraduate degree. He then attended Oklahoma State University of Osteopathic Medicine. He then was selected to Harrisburg, PA. Orthopedic Residency Program at the Osteopathic Community Hospital. He has performed fellowships at not only Shriner’s Hospital for Crippled Children and AO Traveling Spinal Deformity but Texas Back Institute as well where he also served as an attending physician supervising a team of medical students, residents, and fellows. He is board certified by the American Osteopathic Board of Orthopedic Surgery. He is a highly skilled surgeon in both Orthopedic Spine and Extremities. In the spine, he specializes in deformity, arthritis, disc pathologies, and general pain. In extremities, he specializes in Sports Medicine, General Orthopedics, Trauma, and Pediatrics. He has over 15 years’ experience as a trauma surgeon. He is bilingual, speaking fluent English and Spanish.
Dr. Aaron Eubanks, MDOrthopedic Surgeon
Dr. Eubanks specializes in the non-operative, operative, and rehabilitative treatment of various conditions of the bones, muscles, tendons, and ligaments of both adults and children. He obtained his medical degree from New Jersey Medical School in Newark, NJ. After medical school, he completed his residency at Baylor College of Medicine in Houston and his fellowship at Mississippi Sports Medicine. He began his medical career at Sarah Bush Lincoln Health Center, in Mattoon Illinois. He also served as the Team Orthopedist for Eastern Illinois University, Alcorn State University, Hinds Community College, Charleston High School, Hillcrest Christian School, and was the Assistant Team Physician for the Houston Rodeo and Houston ISD football teams. He’s a board-certified, fellowship-trained orthopedic surgeon.

What to expect at your doctor's appointment?
For your initial appointment please arrive 15 minutes before your scheduled appointment. This will allow us sufficient time to complete all of the necessary paperwork.
Please complete the necessary paperwork for new patients.
New patients are requested to bring:
All pertinent medical records, x-rays, and referral paperwork.
A completed copy of the new patient information sheets mailed to all new patients.
Please bring these completed forms to the office at the time of your first visit, do not mail them. Completion of the necessary paperwork prior to your appointment date will increase the efficiency of the visit.
All patients are requested to:
Present all current insurance card(s) at each visit.
Obtain any required insurance referrals depending on plan.
Please dress so that your injury area can be most easily examined and/or x-rayed. For example, wear shorts if you visit for a knee problem, etc.
To cancel an appointment, please let us know at least 24 hours in advance so we can give that time to another patient.
